The Benefits in this reference guide are described according to the following definitions:
FULL-TIME EMPLOYEE – Regularly scheduled to work at least 32 hours per week.
REGULAR PART-TIME EMPLOYEE – Regularly scheduled to work 16-32 hours per week.
TEMPORARY EMPLOYEE – Works on a temporary, seasonal or contractual basis.
POOL EMPLOYEE – Is scheduled to work as needed.
DEPENDENTS–Legal spouse, children under the age of 19, unmarried children up to the age of 25 if permanently residing in the household and supported solely by our employee and grandchildren, if permanently residing in the household and supported solely by our employee.
LICENSED PROFESSIONALS – RNs, LVNs, PTs, OTs, SLPs, LMSWs and Pharmacists.

What You Receive
Health Insurance MM/ You Full-Time Employees and Dependents First of the month following 90 days of full-time employment for non-exempt employees. First of the month following 30 days of full-time employment for exempt employees and licensed professionals. We offer 3 PPO options: one 70/50 plan, one 80/50 plan and one 100/70 plan. Our PPO plans allow you the freedom to choose your own doctor and specialists both in and out of network.
Dental Insurance You Full-Time Employees and Dependents First of the month following 90 days of full-time employment for non-exempt employees. First of the month following 30 days of full-time employment for exempt employees and licensed professionals. Comprehensive dental insurance. Premiums paid by pre-tax payroll deduction. PPO & DHMO plans available.
Vision Discount Plan You Full-Time Employees and Dependents First of the month following 90 days of full-time employment for non-exempt employees. First of the month following 30 days of full-time employment for exempt employees and licensed professionals. Discount program paid by pre-tax payroll deduction.
Life Insurance and AD&D MM Full-Time Employees First of the month following 90 days of full-time employment for non-exempt employees. First of the month following 30 days of full-time employment for exempt employees and licensed professionals. Life insurance and AD&D paid at various amounts based on employment status.
401(k) Plan MM/You All Employees Enrollment occurs at the first of every month. After one year of service, MM matches 50% of the first 4% annually.
Short-Term Disability You Full-Time Employees First of the month following 90 days of full-time employment for non-exempt employees. First of the month following 30 days of full-time employment for exempt employees and licensed professionals. Income protection at low group rates. Premiums paid by after-tax payroll deduction.
Voluntary Term Life You Full-Time Employees and Dependents First of the month following 90 days of full-time employment for non-exempt employees. First of the month following 30 days of full-time employment for exempt employees and licensed professionals. Purchase life insurance for yourself and dependents at low group rates. Premiums paid by after-tax payroll deduction.
Accident Insurance You Full-Time Employees and Dependents First of the month following 90 days of full-time employment for non-exempt employees. First of the month following 30 days of full-time employment for exempt employees and licensed professionals. Provides cash to pay the personal out-of-pocket expenses associated with any accident that occurs while insured under this plan. This direct payment is in addition to any insurance you have.
Cancer Insurance You Full-Time Employees First of the month following 90 days of full-time employment for non-exempt employees. First of the month after 30 days of full-time employment for exempt employees and licensed professionals. Provides cash to pay medical and personal out-of-pocket expenses associated with a current or future cancer diagnosis. This direct payment is in addition to any insurance you have.
Long-Term Disability MM Full-Time Exempt Employees First of the month following 30 days of full-time employment. 60 percent of your salary is paid following 180 days of disability.
Long-Term Care Insurance MM/You Full-Time Exempt Employees First of the month following 30 days of full-time employment. Long-term care is a variety of services that includes medical and non-medical care to people who have a chronic illness or disability.
Telemedicine You Full-Time and Part-Time Employees and Dependents Immediately Reduced-cost health clinic for employees and their immediate family. Membership is required.
Pharmacy You Full-Time Employees and Dependents Individuals who are participants in the MM-sponsored health plan. On-site pharmacy available for employees and their family members with optional payroll deductions.
Paid Time Off MM Full-Time and Part-Time Employees First of the month following 90 days of full-time/part-time employment for non-exempt employees. First of the month following 30 days of full-time employment for exempt employees and licensed professionals. You will accrue 19 days per year to be used for vacation, sick days, personal leave and holidays. Part-time employees accrue at a pro-rated rate based on hours worked.
Employee Assistance Program MM All Employees Immediately Confidential counseling available free of charge to all employees.
Tuition Assistance MM Full-Time and Part-Time Employees (must work 24 hours or more per week) Must be employed at least six months of continuous service. $300 per employee per semester on a first-come-first-serve basis. Reimbursement is for courses only and excludes textbooks, labs, fees, etc.
Employee Compassion Fund You Full-Time and Part-Time Employees After completion of six continuous months of full time or part time employment. Must be a participant in the plan. Benevolent fund for employees that provides reduced interest loans for personal hardship situations.
Funeral Leave MM Full-Time Employees Immediately Up to three days paid absence for death within immediate family (husband, wife, child, parent, sister, brother, mother-in-law or father-in-law).
Jury Duty MM Full Time Employees Immediately The difference between the jury duty allowance and normal daily wage (maximum two weeks).
Leave of Absence MM Full Time Employees Differs based on type of leave requested. Leave of absence without pay unless PTO is available. Return to work is subject to employment conditions at that time. You will be eligible to continue eligible insurance coverage through payment of medical plan premiums.
Direct Deposit You All Employees Immediately Your paycheck is automatically deposited in your account with your signed approval.
Service Awards MM Full-Time Employees Immediately Service awards to employees for each five-year period of continuous service.
Special Events MM/You All Employees Immediately Picnics, parties and other activities planned by each community.
In-Service and Other Educational Opportunities MM/You All Employees Immediately Training on-the-job and other educational programs are available within financial constraints of Morningside Ministries.
